Amy’s Haven accepts several kinds of insurance.

American Specialty Health – Check to see if your company offers this benefit!

No Fault Insurance - Did you know that most automobile insurances cover massage therapy after a car accident?  Avoid chronic pain and other long term complications from untreated car accident injuries.  Ask your physician if massage therapy is a good treatment option for you. For your doctor’s convenience you may print out the massage therapy prescription form below.  Once you receive your prescription, call our office with your insurance and adjustor information and we will pre-verify coverage.   When your insurance coverage is verified you can begin treatment as soon as possible!!!

Worker’s Compensation (Federal and Private) - Did you know that workers compensation covers massage therapy when ordered by your physician?  Massage can be beneficial in soft tissue related injuries, decreasing pain and spasm.  Have you been diagnosed with a work-related injury, low back, shoulder or neck pain?  Ask your physician if massage therapy is a good treatment option for you. For your doctor’s convenience you may print out the massage therapy prescription form below.  Once you receive your prescription, call our office with your insurance and adjustor information and we will pre-verify coverage.   When your insurance coverage is verified you can begin treatment as soon as possible!!!

Injury treatment - Injury treatment usually requires consistent multiple sessions to address soft tissue injury.  Your treatment plan will be tailored to your individual condition.  All treatment plans require a doctor’s prescription and diagnosis as well as treatment frequency and duration.  Feel free to print out the Massage Therapy referral form/Treatment Plan below! 

Health Insurance - Amy’s Haven does not directly bill most health insurance companies (except Kaiser), however, we will provide you with an invoice to submit to your insurance company for reimbursement.  Insurance coverage varies considerably and our services may not be covered by your insurance.  Please call and check your plan.

Special Reports and Documentation fees - If you need special reports or treatment documentation sent to other members of your medical team, for auto accident claims or legal purposes, out staff can provide them although special fees may apply.  No information may be released without a written release from the patient.
